Non distante dalla precedente, questa chiesa custodisce la celebre Madonna dei Pellegrini (1604-06), nata come pala d’altare dedicata alla Santa Casa di Loreto. Alla tradizionale iconografia della Madonna in volo con la casa in cui è nato Gesù, Caravaggio sostituisce la realtà dell’uscio di una casa dal muro sbreccato, a cui si affaccia la Vergine con il Bambino. Posa per questo dipinto Maddelena Antognetti (detta Lena), prostituta d’alto bordo, descritta in un processo in cui è coinvolto il pittore come la “donna di Caravaggio”. Contrasta con la bellezza dei personaggi divini il carattere popolare dei pellegrini, nei cui volti sono ritratti i committenti Cavalletti, devoti alla Madonna di Loreto. L’evidenza dei loro piedi sporchi e callosi suscita tanto scalpore tra i contemporanei di Caravaggio.
